At Harbor Side Care Home / Meadowbrook Manor, we believe families deserve to understand the cost of residential care, what is included, and what financial resources may be available to help support their loved one.
The monthly residential rates listed below reflect the established rates for each room type. However, every resident’s financial situation is different. Before families assume what their personal out-of-pocket responsibility may be, our team can help explore available funding sources, benefits, insurance coverage, and community-based programs for which their loved one may be eligible.
Depending on individual eligibility and care needs, resources may include Medicaid, the Connecticut Home Care Program for Elders, long-term care insurance, Veterans benefits, Medicare-covered healthcare or home health services, and other community-based programs.
Eligibility, coverage, and financial assistance vary by individual, and these programs do not necessarily replace the monthly residential room rate. Our goal is to help families understand the full picture, including the residential rate, what is included, and what additional resources may be available to support their loved one.

24/7 onsite staff, including nights, weekends, and holidays
Medication supervision
Fully furnished accommodations
Free Wi-Fi throughout the building
In-room analog/landline telephone
Resident call buttons located at each bedside and in all bathrooms
Full-building backup generator coverage during power outages
Access to common areas, sun porch, and outdoor spaces
Seasonal activities, arts, crafts, and music programming
Group outings and scheduled day trips
Scheduled transportation for medical appointments and errands
Open family visiting with no restricted visiting hours

What If My Loved One Needs Additional Help?
However, requiring additional assistance does not automatically mean that a resident must leave the home.
When a resident’s needs change, our team works with the resident and their family to explore outside care providers and available programs that may allow additional support to be brought directly into the home.
Depending on a resident’s individual needs and eligibility, these resources may include:

Home health aides
Visiting nurse agencies
Hospital-based or hospital-affiliated home care providers
Medicare-covered healthcare or home health services when eligible
Medicaid-supported services
Private caregivers
Long-term care insurance benefits

Our goal is to explore available resources and support options so residents can continue living safely and comfortably in the home whenever appropriate before families assume that additional out-of-pocket care expenses or a move to a higher level of care will be necessary.

Private Pay
Our team is happy to explain the applicable monthly residential rate, what is included, and what other resources may be available.
Medicaid & State Programs
Eligible residents may also qualify for community-based programs and services that can provide additional support while they are living in a Residential Care Home.
Our team can help families identify potential resources and coordinate with outside providers when additional services are needed.
Connecticut Home Care Program for Elders
Depending on eligibility and individual needs, this may provide another avenue for bringing additional support into the home rather than immediately requiring a resident to transition to a higher level of care.
Our team can help families explore whether this or another outside program may be appropriate for their loved one.
Medicare
However, eligible residents may be able to receive certain Medicare-covered healthcare or home health services from outside providers while living in the home.
Coverage depends on the resident’s individual eligibility, healthcare needs, and Medicare requirements.
Long-Term Care Insurance
We encourage families to review their loved one’s individual policy and contact their insurance provider to determine what benefits may be available.
Our team is happy to assist with supporting documentation when appropriate.
Veterans Benefits
Families who may qualify are encouraged to explore available VA programs. Our team is happy to provide supporting documentation when needed.
